At ACS Home Care LLC, our staff works with each client, along with their physicians, to develop a Care Plan to suit their individual needs. We first help to determine the services required and then work to put together the team to implement the Care Plan.
Each client is overseen by a Registered Nurse to manage their individual Care Plan. The Registered Nurse will:
- Review and assess the client’s needs and abilities on a continual basis
- Integrate the family into the care plan
- Schedule visits by ACS staff members
- Determine if other specialists are required
- Make recommendations based upon the client’s condition and a review of the reports from other disciplines involved
ACS Home Care LLC’s staff works with each client to modify the Care Plan as needed in order to help each client continue to make progress toward attaining the best possible health.
